By completing this form you are requesting to be removed from the prehealth professional student support system including Health Careers Opportunity Program (HCOP) and/or Health Professions Pathways (HPP) Program (if you applied and were accepted to HPP).

By submitting this Opt-Out form you understand that you will no longer be receiving HCOP and/or HPP correspondence, including the HCOP newsletter, with information pertinent to the following career paths:
